Bring your insurance information. If you have Medicare, please bring your Medicare card. We will directly bill the entire cost to Medicare. For private insurance payment, we can bill your insurance company directly. However, you are responsible for deductibles, co-pays according to your insurance contract. For self-pay patients, you are eligible for a 30% self-pay discount. Cash payment $50. Patients should wear short-sleeved shirts.
